Bridge St. Bridgewater NJ 08807 Full-Time Job Description Essential Functions Provides case management to individuals within the Co-occurring And Relationship Empowerment (CARE) program including assessing clients in crisis, providing referrals to appropriate services, working with individuals and staff to develop a recovery plan and program schedule that meets the individual’s recovery goals. Assessing client needs: mental health/substance use/physical health/housing/finance etc. and identify the client’s strengths and challenges. Provides brief support and psychoeducation to clients/families. Collaborate/communicate with systems partners including prisons/jails, DMHAS Interim Managing Entity (IME) to ensure coordination of care. Facilitate treatment plan discussions for clients who are not improving as expected in consultation with the treatment team and who may need more intensive or more specialized care. Facilitate referrals for clinically indicated services outside of the Center (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment, medical specialty care). Documents all clinical activity and correspondence in accordance with Center policy, state regulations, and Joint Commission standards. Participates in supervision and team meetings as well as collaborate with other treatment providers to monitor and discuss clients’ progress and needs. Tracks essential data and performance improvement measures and provide regular reports to the Program Director – CARE. Performs other job-related duties as assigned Qualifications Bachelor’s degree in psychology, counseling, social work, criminal justice, education, or other behavioral health related field Must have relevant experience providing services and case management to individuals living with a substance use disorder Has experience in working across cultures Must possess knowledge of formal/informal community systems and resources Must have knowledge of Wellness and Recovery Principles, Person Centered Care, Trauma Informed Care and Integrated Healthcare Must possess a valid NJ driver’s license in good standing Ability to work collaboratively and build relationships with colleagues, coworkers, vendors, contractors, and the public Preferred CADC/LCADC Bilingual (Spanish/English), and experience working with culturally diverse populations Knowledge of community resources and services and the ability to collaborate with other professionals Trained in conducting Substance Use Interventions Advanced degree in social work, public health, or a related field preferred Benefits include health coverage (medical, prescription, and dental), paid time off (vacation, holidays, and sick), a deferred compensation plan, and enrollment in the NJ State Pension System. #J-18808-Ljbffr
