This role will primarily be remote, operating within the framework of our Hospice Center. Here, you will interface closely with a variety of professionals dedicated to holistic patient care. While working from home, you'll utilize advanced telehealth technology to connect with clients, as well as engage with your team through virtual meetings and collaborative tools. Key Responsibilities\n

• Provide direct clinical services to a caseload of clients with symptoms of major mental illnesses and severe emotional disorders, through both in-person and virtual methods.

• Conduct thorough assessments that are evidence-based and developmentally appropriate, identifying clients’ needs and potential risks.

• Create and implement comprehensive care plans that ensure continuity of care across various services, actively collaborating with family members and other healthcare providers.
\n
• Facilitate individual, group, and family therapy sessions tailored to the unique needs of clients, ensuring a focus on empowerment and choice.
\n
• Monitor client progress, including admission criteria, continuity of care, and discharge planning while maintaining accurate and timely documentation.
\n
• Conduct safety assessments and actively report any concerning situations, ensuring compliance with standards outlined by professional ethical guidelines and organizational protocols.

• Participate in quality improvement committees to enhance agency service delivery and address any barriers to effective patient care.

• Assist with crisis intervention as needed, employing your expertise in crisis management to support clients facing immediate challenges.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited program and current LCSW licensure in New York State.

Compensation and Benefits\n

John Community Health Network offers a competitive salary range of USD 70,000 – 85,000 annually, commensurate with experience and qualifications.
