Healthcare Recruiters seeks an onsite Mental Health Specialist for our clients fast-paced outpatient mental health treatment clinics in Alameda County, CA. No out of state candidates accepted at this time.

Locations: Union City, CA (94587) (Full-Time, Monday–Friday) or Pleasanton, CA (94588) (Part-Time, Tuesday–Thursday)

Our client is an established Medical funded outpatient mental health organization providing specialized services to Alameda County residents for over 25 years. Its high-volume Medication Clinic is seeking a compassionate, organized, and resourceful Mental Health Specialist to coordinate care, engage clients, and support continuity of treatment.

This role serves as a primary point of contact for clients, helping them navigate mental health services, access community resources, and remain engaged in treatment while supporting efficient daily clinic operations.

Key Responsibilities

• Coordinate client intake, admissions, eligibility verification, and required documentation.
• Provide case management, advocacy, referrals, and linkage to mental health, medical, housing, and community resources.
• Build therapeutic relationships and proactively conduct outreach to clients who miss appointments or are at risk of disengaging from care.
• Coordinate services with providers, clinicians, families, case managers, hospitals, and community agencies to support continuity of care.
• Monitor client engagement and treatment goals, assist with discharge planning, and maintain accurate, timely clinical records in compliance with Medi-Cal requirements.
• Support daily clinic operations, patient flow, telehealth services, and a welcoming, organized treatment environment.
• Participate in multidisciplinary team meetings, case consultations, trainings, and quality improvement initiatives.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, Sociology, or a related field; Master's degree preferred.
• Minimum 3–4 years of experience working with individuals with serious and persistent mental illness in a mental health setting.
• Demonstrated experience in case management, client engagement, service coordination, and community resource navigation.
• Familiarity with Medi-Cal documentation, clinical charting, and reimbursement requirements.
• Strong communication, organizational, time-management, and problem-solving skills.
• Ability to work independently, manage competing priorities, respond professionally to crisis situations, and collaborate effectively with multidisciplinary teams.
