About This Role The Home Health-Behavioral Health RN is responsible for admitting patients for home care services, developing individualized care plans, and coordinating with providers to ensure safe delivery of patient care. This role is ideal for nurses with strong clinical skills and the ability to work independently. Highlights Location: New Haven, CT What You'll Do Assess and reassess patients to establish patient-specific plans of care. Collaborate with providers and communicate changes promptly. Assess payment sources and ensure ongoing authorizations. Educate patients and caregivers on medical maintenance and discharge planning. Complete documentation within 24 hours, including changes in condition. Evaluate patient progress and take prompt action as needed. Oversee LPNs and Home Health Aides. Schedule patients according to their individual care plans. Requirements Education: Nursing Diploma or Associate's Degree in Nursing; Bachelor's in Nursing preferred. Experience: Minimum two (2) years of nursing experience in an acute care setting; previous home care experience desirable. Licenses & Certifications: Registered Nurse with a valid (or eligible) Connecticut license required. Valid driver's license and proof of car insurance required.
