Summary Statement:  

The Clinical Manager is responsible for the overall direction of home health clinical services. The Clinical Manager establishes, implements and evaluates goals and objectives for home health services that meet and promote the standards of quality and contribute to the total organization and philosophy. 

 

Responsibilities and Duties:

1. Provides oversight of patient care services and personnel including scheduling, coordinating patient care, coordinating referrals; assuring patient needs are continually assessed: and assuring the development, implementation, and updates of the individualized plan of care.
2. Coordinates and oversees direct and indirect patient services provided by clinical personnel.
   
3. Review referrals and collaborate with intake to obtain necessary documents to admit patients to agency. Promotes home health referrals in the health care community.

4. Provides assistance in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated.
   
5. Maintains communication with attending and referring Primary Care Providers for coordination of care.
   
6. Review OASIS for accuracy and timely completion. Provide education to team as needed to ensure continued compliance with OASISs.
   
7. Oversees compliance audits of documentation and billing.

8. Assists leadership team to oversee QAPI team to ensure high quality of patient care and outcomes.
9. Coordinates clinical data for QAPI indicators and present findings at QAPI meetings in conjunction with QAPI team. Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of home health services provided through the Organization.
   
10. Fields patient concerns/complaints, attempt to resolve or escalate to appropriate leadership staff, as needed.
    
11. Assists in the development of organization goals. Develops, recommends, and administers organization policies and procedures.
    
12. Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and maintains compliance with the Accreditation Commission for Health Care (ACHC) standards. 

13. Along with Clinical Directors, interprets operational indicators to detect census changes and increases or decreases in volume, which could impact staffing levels, revenues or expenses.
    
14. Assists in hiring, onboarding, training, and terminating organizational personnel. Assists with and conducts annual reviews for clinical and intake teams. Ensures appropriate staffing levels for clinical team.
    
15. Assists with developing, implementing, and evaluating the orientation program 
    
16. Plans and implements in-service and continuing education programs to meet education and training needs of organization personnel. Provides guidance and counseling to clinicians to assist them in continually improving all aspects of home health care services provided.
    
17. Oversee competency assessments for all Home Health staff, as directed.
    
18. Complete on-site visits with staff to ensure highest level of competency and provide education.
    
19. Works with Clinical Directors and Administrator to develop strong clinical programming. Develop and grow clinical programming and services.
    
20. Supply management for patients and nursing personnel including ordering and maintaining an adequate supply level; oversight of supply ordering to manage costs.

21. Leads, attends and participates in other department/facility meetings, staff meetings, or other meetings as required, lead weekly interdisciplinary meetings
    
22. Maintain current state licensing, credentialing, and certification. 
    
23. Collaborate with clinical team.
    
24. Provides patient care per Clinical Directors.
    
25. Participates in on-call duties as required, including weekdays, weekends and holidays.
    
26. May require working on holidays/weekends per rotation.

27. Other duties as assigned.

Position Requirements:

1. Registered nurse, Physical Therapist, Speech Therapist, or Occupational Therapist with current licensure to practice in the State of Wisconsin
   
2. A degree in Nursing from an accredited program by the National League for Nursing or minimum bachelor’s degree from an accredited Physical Therapist, Speech Therapist, or Occupational Therapist program.
   
3. Required minimum of two years’ experience in a home health organization.
   
4. Required minimum of 1 year experience in leadership role.
   
5. Current American Heart Association CPR/BLS for Healthcare Providers
   
6. Promote and maintain a collaborative environment.
   
7. Shows ability to organize and prioritize workload independently, multi-task and work in a fast-paced environment.
   
8. Ability to market and deal tactfully with customers and the community
   
9. Excellent attention to detail as well as verbal and written communication skills.
   
10. Knowledge of home health regulations and accrediting agencies.
    
11. Demonstrate ability to supervise and direct professional administrative personnel.
    
12. Will require travel to satellite locations greater than 2 hours, as needed.
    
13. Must be a licensed driver with an automobile that is insured in accordance with state and/or PRN’s requirement and is in good working order.
    
14. Excellent communication and organization skills.

2. Ability to lift 25 pounds from waist height, carry 25 feet, place at floor height, and carry back to starting position/height 

3. Ability to push / pull wheelchair with 30 pounds of force

4. Ability to lift 50 pounds from wheelchair height, stand pivot 90 degrees, and place at waist height 

5. Ability to slide 100 pounds 12 inches up and down bed surface using draw sheet 

6. Ability to lift/turn over 100 pounds using draw sheet 

7. May be exposed to blood borne pathogens (blood or bodily fluids) 

8. Possible exposure to communicable diseases
