Key Responsibilities

• Provide primary care for critically/chronically ill patients after ED discharge through the Roper St. Francis Physician Partners Transitions Clinic locations.

Employment Details

• Employed position with Roper St. Francis Physician Partners Primary Care.
• Salary‑based compensation model.
• Full‑time: Four clinic days plus one admin day per week.
• Part‑time: Two clinic days per week.
• Locations: North Charleston and West Ashley.
