We are seeking an experienced Licensed Clinical Social Worker (LCSW) for a 13-week travel assignment in an acute care hospital setting in Carmichael, California.

Responsibilities:

• Provide psychosocial assessments and interventions for patients and families.

• Conduct crisis intervention and resource planning.
• Participate in patient advocacy and care coordination activities.

Requirements

• Active California LCSW License.
• Current BLS Certification.
• California 5150 Certification required.
• Minimum 2 years of recent acute care social work experience.
• Strong knowledge of discharge planning, crisis intervention, and care coordination.
• Experience with Cerner EMR preferred.
