Job Description
Life Care Centers seeks a Care Manager, Registered Nurse, to coordinate and oversee clinical care for senior residents in a rehabilitation and long‑term care setting. In this role, you will complete assessments, develop and update care plans, monitor resident progress, and collaborate with an interdisciplinary team to ensure safe, high‑quality outcomes. You will educate residents and families, support discharge planning, and help lead quality and compliance initiatives. This position offers a collaborative, patient‑first culture with strong support for professional growth and clinical excellence.
