Key Responsibilities\n

• Perform comprehensive assessments of clients’ mental health needs, crafting individualized treatment plans based on their unique circumstances.

• Maintain open communication with supervisors, family members, and healthcare teams regarding client progress and any necessary adjustments to treatment plans.

• Create and manage a detailed schedule of appointments, including follow-up home visits and virtual check-ins, to monitor advancements and ensure consistent support.

• Conduct home visits as needed and complete thorough chart reviews, ensuring all relevant documentation is up-to-date and accessible.

• Facilitate interdisciplinary team meetings to collaborate on comprehensive care strategies and improve client outcomes.

• Document monthly and quarterly progress notes, providing insights and recommendations based on assessments.
\n
• Lead training sessions for team members, ensuring competency in program implementation and documentation practices.

• Conduct follow-up interviews and in-home observations to assess continued progress and satisfaction with care.

• Act as a liaison with community organizations to strengthen client support networks and develop collaborative care strategies.

• Contribute to the development of training materials that enhance program excellence and client care.

Qualifications\n
