You’ll collaborate with physicians, families, and the interdisciplinary care team to deliver high-quality, individualized therapy services.Key ResponsibilitiesPerform patient evaluations and develop individualized treatment plansProvide therapy for speech, language, cognition, and dysphagiaMonitor resident progress and adjust treatment plans as neededMaintain accurate and timely clinical documentationCommunicate effectively with residents, families, physicians, and care team membersParticipate in care planning and discharge planningContribute to a positive rehab team culture and outcomes-focused careQualificationsDegree in Speech-Language PathologyActive, unencumbered SLP license in South Carolina (required)Advanced degree preferredExperience in a long-term care or rehab setting preferredFamiliarity with PointClickCare (PCC) and Casamba preferredStrong clinical, communication, and organizational skillsCommitment to resident-centered care and professional growthWhy Join Patewood Post Acute?At Patewood, you’re not just filling a role—you’re joining a team that invests in your success and offers real opportunities to grow into leadership.
