SUMMARY OF JOB RESPONSIBILITIES

$5,000 SIGN ON BONUS

Full Time, 4 days a week (80 hrs Per Pay) with rotating weekends - 10 hour shifts. This position is responsible for patient referrals and intake activity; liaison with area discharge planners, referrers, payers and patients/families.

ESSENTIAL FUNCTIONS AND DUTIES (other duties may be assigned)

1. Maintains the confidentiality of patient and employee information at all times, both at work and off duty, and after the termination of employment. According to the agencies Notice of Privacy Practices and HIPAA regulations.
2. Is knowledgeable and functional in computer applications including database, word processing, spreadsheet, and e-mail system as required for the execution of the job.
3. Knowledge of Hospice concept and philosophy.
4. Facilitates hospice referrals by physicians and hospital discharge planners by liaison activity at area hospitals as needed.
5. Assesses and documents appropriateness for hospice at initial assessment visit.
6. Conducts initial nursing assessment visit and arranges for services to begin.
7. Consults with Hospice Physician and/or clinical manager on anticipate needs for new admissions
8. Provides information on hospice care and programs to referral sources and patients and families
9. Assesses patient/family interest in hospice care and services as needed
10. Determines payer source and coordinates the determination of hospice benefits for the referred patient as needed
11. Coordinates with discharge planners to assure continuity of care and patient admission to the hospice program as needed
12. Medication for patient assessed for need and effectiveness on an ongoing basis using comprehensive physical assessment.
13. Works as a team member and collaborates with appropriate co-workers.
14. Demonstrates proper hand washing, asepsis, bag technique and universal precautions.
15. Facilities hospice referrals by physicians and hospital discharge planners by liaison activity at area hospitals as needed.
16. Completes written admission documentation with patients and families as needed
17. Carries out projects and tasks related to community and public relations as assigned

PHYSICAL DEMANDS AND WORK ENVIRONMENT

1. While performing the duties of this job, the employee is frequently required to walk; sit; use hands; reach with hands and arms and stoop, kneel, crouch or crawl
2. The employee is occasionally required to stand.
3. The employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds
4. While performing the duties of this Job, the employee is occasionally exposed to wet and/or humid conditions; fumes or airborne particles and outside weather conditions
5. The noise level in the work environment is usually moderate.

1. Current Michigan License as Registered Nurse.
2. BSN preferred with experience in community based care
3. Possesses and maintains a current CPR certification.
4. Current driver's license.
5. Certification in hospital & palliative care preferred.
