Job Title: Clinical Case Manager Pgm/Dept: CalWORKS! Reports to: Program Manager Classification: Regular, Full-Time, FLSA Non-Exempt Location: 150 Grand Ave., Suite 200, Oakland, CA 94612 Bonita House, Inc. (BHI) is a nonprofit mental health agency with multiple sites in Alameda County and an operating budget of $14.5 million. BHI provides social rehabilitation including a full range of Medi‑Cal services to adults with co‑occurring serious mental health and substance use disorders. Position Summary Under the general direction of the Program Director, the Clinical Case Manager oversees the clinical needs of CalWORKs Phoenix program members. The clinician works within an integrated team, supporting clients’ progress toward greater independence, well‑being, and self‑sufficiency. Services are often provided in office but the manager demonstrates flexibility to meet clients in the community when appropriate, such as library, job, on‑site, and home visits. The role involves maintaining consistent contact, deepening the treatment relationship, assisting clients to identify life goals, and linking them to community resources that support wellness and recovery. The Clinical Case Manager applies motivational interviewing, trauma‑informed care, harm reduction, and abstinence‑based models to teach clients skills for symptom reduction and goal pursuit. Clinical supervision is available. Qualifications Active registration with BBS, Registered MFT, or ASW. Demonstrated ability to work independently and as part of a cohesive team. Ability to prioritize effectively among competing demands. Solid written skills to meet documentation standards. Strong interpersonal skills, including emotional intelligence. Must meet Immigration Reform Act of 1986 requirements. Must have a vehicle, a valid California Motor Vehicle Operator’s License, and automobile insurance. Preferred Bilingual (Spanish) preferred. Experience working with an Electronic Health Record (EHR). Essential Functions / Skills Coordinate clinical case management services for 10‑15 adults referred from CalWORKs Welfare‑to‑Work who have mental health challenges and barriers to employment. Facilitate weekly group activities that promote self‑sufficiency, recovery wellness, healthy boundaries, seeking safety, parenting, spirituality/healing circle, empowerment, literacy, and career development. Provide individual psychoeducation—including symptom management, medication, nutrition, sleep hygiene, trauma‑informed care, and relapse prevention—to clients and family members as appropriate. Conduct initial and annual bio‑psychosocial assessments and establish collaborative treatment goals. Deliver strength‑based, trauma‑informed, culturally humble, non‑judgmental services, linking clients to housing, employment, medical, dental, psychological, parenting, legal, transportation, and money management resources. Familiarity with serving underserved/high‑risk families. Effectively manage and resolve crisis situations. Assist with community and concrete resources. Computer competent with demonstrated ability to learn EHR systems. Complete all clinical documentation within required timeframes. Attend all clinical supervision, case conferences, and team meetings as scheduled. Perform other duties as assigned. Work Environment / Physical Demands The role requires periods of sitting, standing, and walking throughout the workday. Employees may move between offices, program spaces, and meeting areas, including navigating stairs. The position may require travel between agency and partner locations within the Bay Area; reliable transportation is required. Lifting, carrying, or moving items up to 15 pounds occasionally is required. The employee must operate a computer, keyboard, and other office equipment and communicate by telephone and in person. Physical Activity The employee will perform periods of sitting, standing, and walking during the workday. Lifting & Carrying Must be able to lift, carry, or move items up to 15 pounds on an occasional basis. Manual Dexterity & Communication Requires the ability to operate a computer, read printed and digital materials, and communicate effectively in person and by telephone. Travel Required Regular travel to multiple agency and partner locations within the Bay Area is required. Employees must have reliable transportation and the ability to reach designated sites within reasonable travel times.
