Licensed Practical Nurse (LPN) - Woburn Nursing and Rehab Center

Woburn, Massachusetts

Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our nursing team at Woburn Nursing and Rehab Center. As an LPN at our center, you will provide direct resident care, supervise nursing support staff, and ensure high-quality, resident-centered care in accordance with facility policies and regulatory requirements.

Responsibilities:

• Direct the daily activities of Certified Nursing Assistants and other nursing personnel on assigned units
• Ensure compliance with facility policies, procedures, and federal/state regulations
• Participate in admissions, discharges, and transfers of residents
• Observe, document, and report changes in residents’ status to physicians, families, and leadership
• Transcribe and carry out the physician's orders accurately
• Prepare and administer medications and monitor for adverse effects
• Perform wound care, colostomy changes, tube feedings, suctioning, IV administration, and other assigned skilled nursing interventions
• Conduct rounds to ensure resident needs are met, and staff are performing their duties correctly
• Collaborate with the interdisciplinary care team to ensure holistic care for residents
• Initiate, review, and update resident care plans as needed
• Provide end-of-life care as required
• Maintain adequate stock of medications, supplies, and equipment; notify appropriate personnel of needs
• Promote teamwork, mutual respect, and effective communication
• Participate in surveys, inspections, and quality assurance activities

Qualifications:

• Current, unencumbered Massachusetts LPN license
• Strong communication and critical-thinking skills
• Ability to handle situations with tact and professionalism
• Prior long-term care experience preferred but not required
