About the Role

We are seeking an experienced and dedicated Registered Nurse (RN) Clinical Manager to oversee direct patient care services and lead clinical personnel. The ideal candidate will ensure high-quality patient care, regulatory compliance, accurate clinical documentation, and effective coordination of home health services.

Key Responsibilities

• Oversee and coordinate direct patient care services to ensure quality outcomes and compliance with healthcare regulations.
• Conduct and delegate patient assessments and reassessments, update care plans, and evaluate patient needs.
• Determine patient eligibility for admission and assign appropriate clinicians to patient cases.
• Support clinicians in establishing treatment goals, setting priorities, and developing individualized Plans of Care (POCs).
• Coordinate care and communication among clinical staff, physicians, patients, and caregivers.
• Supervise clinical personnel, monitor performance, and provide guidance and professional development.
• Participate in interviewing, hiring, onboarding, and training clinical staff.
• Review clinical documentation, including Start of Care (SOC), Resumption of Care (ROC), and recertification assessments.
• Ensure compliance with Medicare Home Health regulations, including OASIS assessments, documentation, and submission requirements.

Required Qualifications

• Graduate of an accredited School of Nursing.
• Current Registered Nurse (RN) license valid in the applicable state.
• Current CPR certification.
• Minimum of 2 years of RN experience , including at least 1 year of management experience in home care, hospice, or a comparable healthcare setting.
• Hands-on Medicare Home Health experience, including OASIS assessments, documentation, and regulatory compliance.
• Valid driver's license, current auto insurance, and reliable transportation.
