Nurse Practitioner

Optum Home & Community Care, part of the Optum family, is expanding its HouseCalls program to bring care directly to patients' homes in Washington. As an Advanced Practice Clinician (NP or PA), you will conduct in-home health assessments for Medicare Advantage and other plans, with a focus on education, gap closure, and collaboration with PCPs. This role offers flexible scheduling (8am-8pm over a 7-day week), no on-call, weekends, or holidays. #J-18808-Ljbffr
