Part Time In Home Infusions/Injections RN

BrightStar Care is looking for a dedicated and compassionate Part Time RN Case Manager to provide in-home infusion and injection services to patients in the Minneapolis area. This flexible position requires availability 3 days a week between 8:00 AM 5:00 PM, with travel within a 30-mile radius. Mileage is reimbursed. Job Responsibilities:

Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
Collaborate with multidisciplinary team members, partnering pharmacies and providers to optimize patient care and outcomes.
Educate patients and their families on health maintenance and disease prevention.
Ensure accurate documentation and compliance with all regulatory guidelines.

Requirements:

Active RN license in the state of Minnesota.
Previous experience in a home health or acute care setting is advantageous.
Exceptional interpersonal and communication skills.
Committed to delivering high-quality, patient-centered care.
