Care Coordinator

Role: Care Coordinator for a Birth to Three Program, supporting children ages 0–3 with developmental delays or disabilities and their families. Responsibilities Include Conducting functional assessments and creating individualized service plans Coordinating and facilitating family-centered services and team meetings Partnering with families, service providers, and community resources Providing education, advocacy, and support to families Maintaining accurate documentation and ensuring regulatory compliance Managing a flexible schedule, including home visits and occasional evening or weekend hours Qualifications & Skills Bachelor's degree in Social Work, Child Development, or related field Preferred: one year of supervised experience working with families with special needs Valid driver's license, reliable transportation, and ability to meet driving requirements Strong communication, organizational, and documentation skills Comfort working in home, office, and community settings Ability to respond effectively in crisis situations Work Environment: Community-based with daily travel, moderate physical activity, and exposure to diverse environments.
