Requirements Start- 11/28/2026 WA license in hand at time of present not open to IMLC at this time due to fast rack or urgent credentialing timeframe to meet start MD/DO Behavioral Health FTE- 1 Why Open? Gap in coverage- current locums provider not extending after August Location- Both Inpatient outpatient settings located 5 minutes from each other Both Clinic Hospital privileges needed Rounding Admissions Yes discharges are required as well Demographics- Adults Avg. patients per day 12-14 Hospital bed size: 126 Required Procedures- N/A Clinical Activity past 24months- Applicant must provide documentation case list of provision of clinical services 100 patients representative of the scope and complexity of the privileges requested during the previous 24 months. Case list must include diagnosis or procedure and date and facility where performed. Provider will be working Solo- Yes Licensed Candidate- Yes WA in hand at time of presentation is required Certs- BLS EMR- EPIC Shift Info: Day 5x8-Hour 08:00 - 17:00
