What You'll Do

• Evaluate and treat patients with physical functioning disorders, including wound care, following clinical standards and best practices
• Develop and implement personalized care plans using evidence-based assessments and interventions
• Administer procedures such as manual techniques, therapeutic exercises, modalities, and assistive device use
• Monitor patient progress and modify treatments accordingly, documenting all care activities accurately in electronic records
• Collaborate with healthcare team members on patient care and discharge planning
• Provide consultation and education to patients, families, and caregivers
• Support supervising and mentoring rehabilitation aides, students, or assistants as needed

What You Need

• Valid State Physical Therapist license
• Minimum of 1 year of relevant experience in rehab or outpatient settings preferred
• Strong communication skills and ability to work collaboratively
• Knowledge of clinical documentation practices and regulatory standards
