Advanced Practice Provider- Community Care

Overview The Nurse Practitioner (NP) is responsible for the provision of health‑care services to individuals, families, and/or groups who receive their care under the auspices of Sentara. Health‑care services include conducting health assessments, prescribing/providing treatments/interventions, promoting health, and providing disease prevention and management. Health‑care services are provided under the direction and supervision of an assigned primary care physician (collaborative agreement). The Nurse Practitioner is expected to participate in quality assurance and improvement activities, engage in team activities, and use resources (human and material) appropriately. Location City/State: Norfolk, VA Work Shift: First (Days) Provider Specialty: Community Care Job Details In this role you will be working between Union Mission and the Sentara Mobile, serving patients through the men’s shelter, women’s shelter, and partnering with Sentara behavioral health. Hours: 36 patient hours per week, alternating between Union Mission and Mobile, plus 4 hours of admin. Education: Master’s Level Degree – Nurse Practitioner. Certification / Licensure Registered Nurse (RN) Multi State – Nursing License – North Carolina Board of Nursing (NCBON) Registered Nurse (RN) Single State – Nursing License – North Carolina Board of Nursing (NCBON) Registered Nurse (RN) Multi State – Nursing License – Virginia Department of Health Professionals (VADHP) Registered Nurse (RN) Single State – Nursing License – Virginia Department of Health Professionals (VADHP) Advanced Practitioner Registered Nurse (APRN) – Nursing License – Virginia Department of Health Professionals (VADHP) Licensed Nurse Practitioner (NP) – Nursing License – North Carolina Board of Nursing (NCBON) BLS (Required) Qualifications ACLS (Preferred) 2-3 years of minimum NP experience required.
