Job Description

Make a meaningful impact in patient lives by providing quality care directly in their homes. This full-time, 13-week contract opportunity empowers registered nurses to put their skills to work in a field-based setting, focusing on individualized care and patient education.

Schedule:

• Monday Friday
• 8:00 AM 5:00 PM

Core Qualifications:

• Current, active RN license in good standing (state of practice)
• Minimum 1 year of recent home health nursing experience (field experience highly preferred)
• Proficient with Home Care Home Base (HCHB) electronic documentation
• Demonstrated expertise with OASIS assessments and care planning
• Reliable transportation, valid drivers license, and current auto insurance

Key Responsibilities:

• Complete in-depth OASIS assessments and create tailored care plans
• Deliver skilled nursing services such as wound care, IV therapy, medication administration, and chronic disease management
• Visit 35 patients per day within an assigned territory, with a target of 30 points per week
• Closely monitor patient progress and communicate changes to care teams and physicians
• Provide education to patients and families regarding treatment, disease processes, and prevention
• Accurately document all care in HCHB and participate in care conferences, meetings, and continuing education
• Collaborate with therapists, social workers, and aides to ensure coordinated care delivery

$2,057 - $2,176/WeekEstimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
