Physical Therapist

Schedule: M - F 8am to 4pm with 2-3 weekend days

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 37

Requirements:

• Valid state license to practice physical therapy
• Appropriate certifications as required by regulations
• Experience in home health or related field is preferred

Roles & Responsibilities:

• Actively monitor and adjust patient visit schedules to maximize productivity and meet patient needs, including rescheduling and communicating changes to supervisors.
• Conduct comprehensive assessments of assigned patients, covering psychosocial, educational, safety, and risk factor parameters, in accordance with all relevant regulations and guidelines.
• Develop individualized plans of care that promote optimal functional independence and ensure documentation aligns with standard of care and regulatory requirements.
• Provide skilled physical therapy treatment, including therapeutic techniques and patient education, while documenting progress and maintaining quality of care.
• Coordinate patient care with a multi-disciplinary team to ensure effective communication and optimal treatment outcomes.
• Supervise and direct the clinical activities of physical therapist assistants, students, and home care aides.
