Primary Care Nurse PractitionerProgram: Healthcare Department, Mobile ClinicsWork Location: Varies; sites are located in the Bronx, Manhattan, and BrooklynStatus: Part TimeSchedule: Monday, Thursday, and Friday 9-5Reports to: Medical Director, Primary CareSalary: $80,000 to $87,000 depending on experiencePosition Overview:The Primary Care Nurse Practitioner working in Mobile Health is responsible for the examination, diagnosis and the treatment of patients within the scope of the Health Center. It is preferred the candidate has experience working with patients experiencing homelessness, as well as those with substance use disorders and serious mental illness. Experience providing care in a mobile setting is a plus. This posting is for a part time Family Nurse Practitioner, working three days a week.Essential Duties & Responsibilities:The essential duties of the Primary Care Nurse Practitioner include but are not limited to the following activities:Provides medical diagnoses, treatment and advice to patients in accordance with New York State regulations and the scope of training of the clinician.Responsible for consulting with the appropriate medical specialists, when deemed necessary according to the Health Center protocol.Prescribes and/or dispenses required medication to patientsProvides medical supervision to the assigned clinical team leadersDirects, organizes and participates in preventive health programsParticipates in the development and implementation of quality assurance management and utilization reviewParticipates in interdisciplinary meetings with shelter and housing staff for the assigned site to discuss high risk patientsParticipates in quality improvement initiativesParticipates in community outreach programs as neededContributes to the overall functioning of the departmentMaintains patient records in accordance with the policies of the departmentMaintains all New York State and federal licensing requirements as current according with the department's credentialing policyMaintains current certification in Basic Life SupportCompletes 30 annual Continuing Medical Education credits.Participates in Departmental meetings and conferences.Participates in on call coverage on a rotating basis, taking a total of 4 weeks of telephone call coverage per year.Qualifications:New York State NP License and RegistrationFamily Nurse Practitioner Board CertificationExperience in providing services to homeless and substance use population preferred; mobile health experience is a plusDemonstrated knowledge of health related concerns and experience in providing medical care in urban communitiesCommitment to community-based health care servicesBilingual (Spanish/English) preferredCommitment to the mission of Project RenewalProject Renewal is an equal opportunity employer.
