Central Intake Coordinator

The Pavilion is seeking a Central Intake Coordinator to support admissions and referral management operations for the organization. This role is responsible for reviewing incoming referrals, coordinating admissions communication, and working closely with facility leadership to ensure efficient intake processes. The ideal candidate understands the skilled nursing environment and can manage high referral volume in a fast-paced setting. Key Responsibilities Review and process incoming referrals from hospitals and referral sources Coordinate with facility clinical teams regarding admissions acceptance Communicate with case managers, discharge planners, and referral partners Track referral activity and maintain accurate intake documentation Support census growth through efficient intake coordination Collaborate with Admissions Directors and leadership teams across facilities Ensure timely follow-up on pending referrals and admissions decisions Qualifications Prior admissions, intake, or referral management experience required Skilled Nursing Facility or post-acute care experience strongly preferred Strong understanding of Medicare, Medicaid, and managed care processes Excellent organizational and communication skills Ability to manage multiple referrals and priorities simultaneously What’s Being Offered Competitive compensation aligned with experience Stable leadership environment Opportunity to work with a growing organization Long-term growth potential within the company IND456 #J-18808-Ljbffr
