Job Objective To provide, supervise and coordinate skilled nursing care, personal care and other services given to her/his assigned patients. The RN performs comprehensive patient assessment, develops and oversees the execution of the plan of care, ensures care is provided as ordered, delegates nursing services to Home Health Aides (CHHA) and Licensed Vocational Nurses (LVN), and provides skilled nursing care. Duties and Responsibilities Act as the patient’s advocate and protect patient confidentiality. Distribute and explain patient’s rights and responsibilities. Demonstrate knowledge regarding special needs and behaviors of age‑specific groups and age‑specific competencies. Understand medical disease processes, surgical procedures, psychosocial behavior, and nursing theory and practice. Conduct initial nursing assessment covering physical, physiological, psychosocial status, therapeutic regimen, and environmental factors. Perform patient care in accordance with the plan of care and MD orders. Provide counseling and instruction to patients and caregivers to meet nursing and related needs. Coordinate care with physicians and other interdisciplinary staff; participate in case conferences. Encourage and facilitate patient/caregiver self‑management of care. Initiate and revise treatment plans based on assessment; establish goals, discharge plans, and routinely re‑evaluate treatment plans. Apply best practices and disease‑management programs as available. Communicate effectively with co‑workers, managers, office staff, case managers, physicians, and the Director of Patient Care Services. Maintain comprehensive, clear, and timely documentation of nursing interventions, patient responses, and coordination with other services. Participate in age‑specific educational and training programs. Overall planning, ongoing care, and discharge planning of patients, ensuring compliance with regulations and authorization of payment. Notify physicians of plan of care changes or barriers and maintain communication with all disciplines. Maintain accurate schedules, manage lab requests, recertification, and supervision of CHHA/LVN as per agency policies. Supervise CHHA staff through care planning and bi‑weekly supervisory visits, and supervise LVN staff through care oversight, communication, and quarterly supervisory visits. Non‑Essential Duties Participate in Quality Improvement (QAPI), utilization review (UR), chart audits, and performance improvement committees. Support and mentor nursing students and other healthcare trainees. Orient new RN, LVN, and CHHA staff. Perform other duties as assigned. Knowledge, Skills and Abilities Knowledge of the Nursing Process: assessment, planning, and supervision of patient care within scope of practice. Familiarity with agency policies, procedures and practices. Compassionate bedside manner and ability to provide care with compassion and caring. Effective oral and written communication with case managers, DPCS, patients, and caregivers. Strong assessment, clinical, and critical‑thinking skills. Basic computer literacy. Organizational skills to maintain visit schedules and paperwork. Ability to connect with patients and families in their homes in a kind manner. Professionalism in all aspects of patient care. Independent problem‑solving with appropriate supervision. Supervision of LVN and CHHA staff as required. Comprehensive understanding of Home Health regulations and financial implications of care. Knowledge of compliance, quality assurance, and improvement guidelines. Education and Experience Graduated from an approved school of nursing; BSN preferred. Minimum of 1 year recent relevant clinical experience as an RN in an acute or subacute setting (e.g., SNF, long‑term care). Home Health experience preferred; strong clinical skills required for independent practice. Required Licensure and/or Certifications Current RN license in California. Current CPR certification. Valid California driver’s license and evidence of current auto insurance. AVMC Conduct/Compliance Expectations Adhere to attendance, punctuality, leaves of absence, and PTO policies.
