Clinical Pharmacy Specialist - PRN (As Needed)

Position Overview The Clinical Pharmacy Specialist will serve as the pharmacy representative in all aspects of care within their area. Responsibilities include reviewing pharmacy and organizational policies regarding drug use, ensuring readiness for regulatory audits, overseeing formulary management and drug‑related education, and collaborating with hospital departments to evaluate and modify pharmacy services. The role involves training clinical staff, overseeing pharmacy team members on the unit, engaging in academic scholarship (authorship, presentations, attendance at national meetings), and developing collaborative practice opportunities within Children’s National. Qualifications Minimum Education Doctor of Pharmacy (Pharm.D.) (Required) Minimum Work Experience 2 years of pharmacy experience in a pediatric, specialized, academic, or clinical setting (Required) 6 years required for candidates without a PGY2 residency (Required) Required Licenses and Certifications Pharmacist eligible for licensure in the District of Columbia plus/minus Maryland and Virginia (specialty dependent) (90 Days Required) Board Certified Pharmacist (BPS) (specialty dependent) (1 Year Required) Functional Accountabilities Order Processing and Accreditation Readiness Interpret medication orders, oversee packaging, labeling, and distribution of medications. Input orders into the pharmacy computer system when appropriate. Staff the pharmacy operation to maintain distribution skillset. Assist in performing unit inspections and verifying accreditation readiness. Review medication lists for therapeutic duplication, omissions, dosing errors, drug interactions, and allergies. Verify new medication orders for appropriate drug, dose, route, and frequency. Assist nursing staff and other providers with automated dispensing machine questions. Interventions and Documentation Perform drug therapy monitoring and intervene on drug dosing, administration, interactions, side‑effects, and monitoring issues. Complete therapeutic drug monitoring consults and adverse reaction reports. Document interventions, drug information, and chart entries where applicable. Collaborate with the multidisciplinary team to ensure appropriate medication access, including alternative therapies. Participate actively in medication reconciliation to ensure a complete and accurate home medication list. Work with providers to implement comprehensive medication evaluation and management across care settings. Staff Development and Training Assist with education, training, and general supervision of pharmacy students and residents. Assist with training new pharmacists. Provide lectures and drug reviews to professional groups and committees. Develop system changes, educational programs, and interventions that reduce prescribing, dispensing, and administration errors. Clinical Services Ensure appropriate medication use organization‑wide. Identify drugs/therapies for further evaluation and formulary addition. Provide accurate and timely drug information to physicians, nurses, and other health professionals. Maintain an evidence‑based clinical practice. Serve as a clinical specialist resource for pharmacists, nurses, and physicians; address and follow up on system and patient care issues. Assist the department in managing drug shortages. Represent the department on key committees. Assist pharmacy operations and informatics teams in implementing new and updating existing formulary items. Become credentialed and help design, implement, and execute collaborative practice agreements. Quality Improvement Lead cost‑savings initiatives within the department. Lead and participate in quality improvement initiatives both within the department and externally. Contribute to development of order sets and electronic order set builds. Assist with growth of clinical pharmacy programs throughout the organization. Actively engage in performance, quality improvement, and patient safety activities for the service area, Pharmacy Services, and the Enterprise. Identify high‑cost, high‑use agents and participate in cost‑management initiatives. Professional Development Participate and/or lead research initiatives with medical unit and/or resident projects. Attend, present, and moderate at subject‑area conferences. Engage in preceptor development seminars and teaching in Children’s preceptor development series. Publish in primary literature; deliver poster or oral presentations. Organizational Accountabilities Anticipate and respond to customer needs, ensuring follow‑up until needs are met. Demonstrate collaborative and respectful behavior; partner with team members; be receptive to others’ ideas. Contribute to a positive work environment; demonstrate flexibility and willingness to change. Identify opportunities to improve clinical and administrative processes; make appropriate decisions using sound judgment. Use resources efficiently and seek less costly alternatives.
