Job Overview - Occupational Therapist:
Join our client's team as an Occupational Therapist in Brooklyn, NY. In this full-time role, you will design and implement therapy treatment plans to restore function, alleviate pain, and prevent disabilities for medically referred participants. Working within an interdisciplinary team, you will make a meaningful impact in both clinical and community settings.

Compensation: $60/hour
Location: Brooklyn, NY
Schedule: Monday to Friday, 9:00 am - 5:00 pm

Responsibilities as the Occupational Therapist:

• Participant Evaluation: Conduct assessments and develop treatment plans at PACE sites, homes, or nursing facilities.
• Care Planning: Collaborate with interdisciplinary teams to create and update individualized care plans.
• Documentation: Maintain accurate records, including evaluations, progress notes, and care plans, per policies.
• Education and Training: Provide clinical instruction to staff, students, and volunteers and lead training programs.
• Discharge Planning: Recommend equipment and coordinate smooth participant transitions.
• Collaboration and Safety: Consult with healthcare professionals, ensure safety standards, and provide education during community visits.

Qualifications for the Occupational Therapist:

• Education: Successful completion of a Physical Therapy program from an APTA-accredited university.
• Licensure: Current New York State licensure as a Registered Occupational Therapist is required.
• Experience: At least 1 year of experience as an Occupational Therapist, preferably with geriatric or frail populations.
• Skills: Proficient in using OT modalities and equipment, with the ability to establish rapport with patients and staff.
• Technical Proficiency: Familiarity with PACE regulations, Care Compass, and other EMR systems.
• Attributes: Strong organizational skills, attention to detail, and a commitment to patient-centered care.
• Travel: Must be able to travel to participant homes, nursing facilities, and clinics.
• Language Skills: Bilingual candidates are strongly preferred.
