Responsibilities
- Conduct initial and ongoing nursing assessments to identify patient needs and develop an individualized plan of care aligned with goals and policies.
- Manage a patient caseload, coordinating with physicians and other members of the care team to implement and adjust the plan of care.
- Document patient status, responses to treatment, and plan changes in the electronic health record (EPIC), ensuring timely, accurate charting.
- Educate patients and families about care plans, rights and responsibilities, and self-management, providing written materials as appropriate.
- Collaborate with interdisciplinary teams to facilitate safe, effective care transitions and address barriers or problems as they arise.
- Ensure compliance with legal and regulatory requirements, maintain HIPAA standards, and uphold professional nursing practice.

Requirements
- Active Registered Nurse (RN) license in the state of practice
- CPR/BLS certification (Required)
- Minimum two years of recent RN experience in acute care or case management
- EPIC experience required
- BSN preferred
- Valid driver's license and current automobile insurance; reliable vehicle
- Ability to work in a hospital/short-term acute care setting on a travel assignment
