ECM Case Manager: Wellness & Care Coordination
AACI
AACI is seeking an ECM Case Manager in San Jose, CA, for full-time engagement. You will outreach, assess high‑risk clients, and coordinate with a multidisciplinary team to link clients to wellness and recovery resources.
Responsibilities include developing individual care plans, facilitating referrals, and supporting community resources. English fluency is required; bilingual skills are a plus. Driving is required for field work.
