Key Responsibilities\n

• Conduct thorough evaluations and assessments of seniors to determine their psychosocial needs.

• Collaborate with multidisciplinary teams to develop and implement Individualized Family Service Plans (IFSP) tailored to the needs of each senior client.

• Monitor client outcomes through regular follow-ups and adjust treatment plans as necessary, ensuring the highest level of care.

• Provide support and counseling to families, fostering communication and understanding to promote a healthy family dynamic.

• Ensure compliance with all federal and state regulations related to social work practices.
