This position is fully remote, allowing you the flexibility to conduct assessments and provide support without the constraints of a traditional office setting. You will primarily communicate with clients and their families via video calls and phone consultations, ensuring you remain accessible to those you serve. Key Responsibilities\n

• Conduct in-depth assessments to evaluate seniors' psychosocial needs, focusing on emotional and social factors that impact their well-being.

• Work closely with clients and families to co-create personalized care plans that reflect their goals, preferences, and cultural values.

• Provide one-on-one counseling, offering emotional support as clients navigate the complexities of aging, such as health changes and family dynamics.

• Advocate vigorously for clients’ access to services and resources within the community, ensuring they receive appropriate assistance.

• Collaborate with interdisciplinary teams including nurses, caregivers, and other health professionals to ensure cohesive care delivery.

• Engage with clients regularly through follow-up visits or virtual check-ins to monitor progress and make necessary adjustments to care plans.

• Facilitate family communication and conflict resolution to foster a supportive caregiving environment for seniors.

• Document all interactions, assessments, and care plans in electronic health records (EHRs) to maintain comprehensive and accurate client histories.

Qualifications\n

Compensation and Benefits\n

We offer a competitive salary range of USD 60,000 to 100,000 annually, commensurate with experience and qualifications.
