We are seeking an experienced and compassionate Care Manager (MSW) to join our team and serve as a vital resource for new and existing clients. This position is ideal for a social worker who enjoys connecting with families, conducting assessments, coordinating services, and helping clients successfully navigate the continuum of care.

Care Manager Overview

The Care Manager serves as the first point of contact for prospective clients and their families. This role is responsible for conducting intake assessments, completing in-home consultations, gathering client information, developing care recommendations, and assisting clients and families with care navigation and community resources.

Success in this role requires exceptional time management, organization, communication, and follow-through. The ideal candidate is comfortable managing multiple referrals, prioritizing tasks, and maintaining accurate documentation while delivering a high level of customer service.

Care Manager Responsibilities

• Conduct intake assessments with prospective clients and family members.

• Complete in-home consultations to evaluate care needs, safety concerns, and support requirements.

• Gather and maintain comprehensive client information and documentation.

• Develop care recommendations and coordinate service initiation.

• Assist clients and families with care navigation, education, and referrals to community resources.

• Collaborate with caregivers, healthcare providers, hospitals, senior living communities, and family members.

• Conduct new caregiver introductions.

• Monitor client needs and provide ongoing support and follow-up.

• Maintain timely and accurate documentation within the agency systems.

• Participate in an on-call rotation to provide after-hours support and guidance to clients, families, and caregivers as needed.

• Build trusting relationships with clients and families throughout the care journey.

Care Manager Qualifications

• Master of Social Work (MSW) preferred; Bachelor of Social Work (BSW) will be considered

• Previous experience in care management, case management, home care, healthcare, geriatrics, or social services preferred.

• Strong assessment and problem-solving skills.

• Excellent interpersonal, verbal, and written communication abilities.

• Outstanding organizational skills with the ability to manage competing priorities.

• Proven time management skills and attention to detail.

• Ability to work independently while collaborating effectively with a team.

• Valid driver's license and reliable transportation for home visits and introductions.
