Physical Therapist (PT) Home Health

We are currently hiring a Physical Therapist (PT) to provide skilled therapy services to patients in their homes throughout Leesburg and surrounding areas. Responsibilities Perform physical therapy evaluations and develop individualized treatment plans Provide skilled therapy, including therapeutic exercises, gait training, balance training, and mobility interventions Educate patients and caregivers on exercises, safety, fall prevention, and assistive devices Monitor patient progress and adjust treatment plans as needed Complete timely and accurate clinical documentation Communicate with physicians, nurses, and the interdisciplinary care team Supervise PTAs as required Qualifications Graduate of an accredited Physical Therapy program Current Florida PT license in good standing Minimum 1 year of PT experience preferred Current CPR certification Valid driver's license, reliable transportation, and auto insurance Ability to meet all required background and compliance requirements Why Join Elite Home Health?
