In-Home Clinical Assessment Manager
Matter of Care Home Health Services
Matter of Care is seeking a Clinical Assessment Manager to perform comprehensive in-home assessments using UAS-NY, identify care needs, and connect older adults with services to support independence. This role is part of the CenterLight Health System family. You will develop service plans, collaborate with care teams, document findings in EMR, and travel within The Bronx and Manhattan as needed.
