LPN/LVN Case Manager: Care Coordination & Advocacy
PACS
PACS in California seeks a licensed LPN/LVN to work with residents, families and providers to develop and implement care plans that deliver extraordinary patient care. The candidate must hold a current California LPN/LVN license (RN acceptable) and have at least two years of clinical nursing experience; familiarity with Medicare, Medi-Cal and Medicaid programs is a plus.
