Hospice RN Case Manager — Home-Based Patient Care

Anchor Health

Anchor Health in California is seeking a Hospice Registered Nurse to perform comprehensive patient and family assessments, develop and revise plans of care, and deliver in-home nursing services with a focus on comfort and quality of life. You will administer medications, participate in interdisciplinary rounds, document care, and mentor ancillary staff, with driving to patient homes required. California RN license, CPR, and hospice/home health experience preferred. #J-18808-Ljbffr Anchor Health
