About the Role

As an Operation Home Case Manager, you'll work directly with individuals and families experiencing homelessness, chronic housing instability, and other barriers to stable housing. Your role goes beyond helping someone find a place to live - you'll walk alongside clients as they work toward maintaining housing, increasing stability, building independence, and creating a path toward long-term self-sufficiency.

You'll manage a caseload of approximately 20–25 clients, developing individualized plans based on each household's unique needs and goals. You'll connect clients with community resources, advocate for benefits and supportive services, help navigate housing searches, collaborate with landlords and community partners, and provide ongoing support once housing is secured.

This is a relationship-driven role for someone who can balance compassion with accountability. You'll celebrate progress with your clients while also helping them navigate setbacks, overcome barriers, develop practical life skills, and stay focused on the goals they've created for themselves.

Not every day will look the same. You may spend part of your day completing case notes and coordinating services, then head into the community for a home visit, connect with a landlord, collaborate with another provider, or help a client navigate an unexpected crisis.

What You'll Do

• Manage a caseload of approximately 20–25 individuals and families, depending on program assignment
• Help clients secure and maintain stable housing while connecting them with supportive services and community resources
• Conduct comprehensive assessments to understand each client's strengths, needs, goals, and barriers to long-term stability
• Partner with clients to develop individualized action plans focused on housing, health, income, financial stability, and other essential needs
• Advocate for clients as they navigate benefits, entitlements, supportive services, and community resources
• Support clients through housing searches and help identify safe and affordable housing options
• Serve as a liaison between clients and landlords and assist with lease agreements, utility arrangements, and housing-related concerns
• Conduct monthly home visits and case reviews to monitor progress, identify emerging needs, and support continued housing stability
• Provide immediate support during client crises, including safety planning and connections to emergency services when necessary
• Help clients strengthen financial independence through budgeting, saving, debt management, and financial literacy
• Collaborate with workforce, SOAR, nutrition education, housing, and other supportive service providers
• Build and maintain strong relationships with community partners and referral networks
• Case conference with internal and external partners to coordinate services and identify creative solutions for complex situations
• Continuously reassess client goals and service plans and adjust strategies when needs or circumstances change
• Maintain accurate, thorough, and timely case documentation
• Enter services, referrals, recommendations, case notes, and required program documentation into HMIS and Salesforce within one business day of client contact
• Maintain required forms, reports, shared tracking tools, and program documentation
• Prepare monthly client success stories for leadership, Marketing, and Grants
• Stay informed about community resources, eligibility requirements, application processes, and best practices for serving diverse populations
• Represent The Just One Project at organization-wide events, including participation in up to six Pop-Up & Give events annually
• Support additional program and organizational responsibilities as needed

What You Bring

• Bachelor's degree in Social Work, Human Services, or a related field
• 3+ years of nonprofit experience
• 3+ years of experience in case management, social services, or community outreach
• Experience working directly with diverse populations, including individuals who may be vulnerable or at risk
• Relevant certifications in case management and conflict resolution
• Experience managing a caseload and helping clients achieve measurable outcomes related to stability and self-sufficiency
• Familiarity with housing services, wrap-around programs, and self-sufficiency initiatives
• Strong relationship-building skills and the ability to establish trust while maintaining professional boundaries
• The ability to approach clients with empathy, dignity, respect, and a client-centered mindset
• Strong problem-solving and conflict-resolution skills, especially when navigating complex client needs or housing barriers
• Excellent organization and time-management skills with the ability to manage multiple priorities and deadlines
• Strong documentation and data-entry skills
• Comfort using client management and tracking systems such as HMIS and Salesforce
• Knowledge of community resources and the ability to connect clients with appropriate services
• Ability to coach clients in practical skills such as budgeting, financial planning, and resource navigation
• Ability to obtain and maintain a valid Nevada Food Handler Safety Training Card within 30 days of hire

Preferred Qualifications

• Familiarity with SNAP program procedures and requirements
• Fluency in both English and Spanish

Schedule & Community-Based Work

This is a full-time position scheduled for approximately 40 hours per week, Monday through Friday from 8:00 a.m. to 4:30 p.m.

This isn't a fully desk-based case management position. Regular travel throughout the Southern Nevada community is an essential part of the role, including monthly visits to clients in their homes.

Candidates must have a valid, unrestricted driver's license, reliable personal transportation, and the ability to meet the organization's insurability requirements.

Evening, weekend, event-based, or on-call work may occasionally be required depending on operational needs.

• Working independently in community settings without direct supervision
• Driving regularly throughout Southern Nevada
• Entering a variety of client home environments
• Navigating stairs and tight or cluttered spaces
• Remaining calm and professional in high-need, complex, or unpredictable situations
• Sitting, standing, walking, reaching, lifting, and carrying throughout the workday
• Lifting and carrying up to 50 lbs. as needed
• Working in varying indoor and outdoor environments
