Occupational Therapist - PRN - The Garrison Care Center - Lubbock, TX Join Our Team and Start Making a Difference! The Garrison Care Center in Lubbock, TX , is a skilled nursing facility with long term care, post-acute rehab, and geriatric outpatient therapy services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a PRN Occupational Therapist for our in-house rehab program! Key Responsibilities Effectively screen and evaluate patients with cognitive and ADL physical functioning disorders, creating individualized treatment plans that meet regulatory and clinical practice standards. Utilize evidence-based practice and standardized assessments to support clinical interventions. Provide rehabilitative, skilled treatment interventions including ADL retraining, therapeutic activities, and assistive device training. Assess patient progress and adjust treatments as necessary to meet goals. Work closely with families, caregivers, and the healthcare team to ensure comprehensive patient care. Communicate progress effectively with the healthcare team and document outcomes according to regulatory standards. Provide discharge summaries and recommendations for continued care.
