Home Health Licensed Practical Nurse LPN

Licensed Practical NurseThe Licensed Practical Nurse is responsible for assessing patient health problems and needs, implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration is required.Essential Duties And ResponsibilitiesThe essential functions include, but are not limited to the following:Provide skilled nursing care under the direct supervision of the Clinical SupervisorComplete evaluation tasks, including vital signs and medication reviewsCoordinate with physicians, therapists and other individuals in the client’s care planAssess and chart observations of the client’s condition at each visitMaintain complete and accurate documentation of nursing careWork collaboratively with all members of the healthcare teamProvide education to clients and families on proper home health care procedures and strategiesRecord patients' medical information and vital signs.Monitor, record, and report symptoms or changes in patient's conditions.Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plansModify patient treatment plans as indicated by patients' responses and conditionsMonitor all aspects of patient care, including diet and physical activityAssess the needs of individuals, families, or communities, including assessment of individuals' home or work environments, to identify potential health or safety problemsPerform other duties as assignedMinimum Qualifications (Knowledge, Skills, And Abilities)Associates degree required; Graduate of an accredited state-approved, Bachelor’s Degree from school of nursing preferredCurrent and valid Connecticut Licensed Practical Nurse licenseOne year of LPN experience, Current CPR certification (required)Driver’s License, Car Insurance, Car RegistrationStrong interpersonal skills that build personal rapport and establish trust with the clientComputer literate, Strong organizational skillsExcellent written and communication skillsAbility to meet physical demands of the job (ex: lifting 50 pounds for transfers)Home health care experienceBilingual (Spanish) preferredPhysical Demands And Work EnvironmentThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
