Your Role and Responsibilities

• Perform in-depth patient assessments, order necessary diagnostic tests, and create personalized care plans
• Diagnose and manage acute, chronic, or episodic conditions in partnership with physicians or independently based on your credentials
• Guide patients and their families on wellness, preventive care, and health management
• Prescribe medications and therapies aligning with your professional scope
• Engage in follow-up and chronic care management
• Work collaboratively with interdisciplinary teams to provide cohesive and comprehensive patient care
• Maintain meticulous documentation in electronic medical records (EMR)

Qualifications We Seek

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) required
• Successful completion of an accredited Nurse Practitioner program

Mandatory Certifications

• Active Nurse Practitioner license required in the state of Arizona
• Certified as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or equivalent specialty
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications, or ability to acquire within 90 days
