Hospice Registered Nurse Case Manager

Trinity Health At Home is seeking a Hospice RN Case Manager in Michigan to provide professional nursing care for hospice clients as prescribed by the physician and in compliance with state regulations. The role covers the RN’s assessment, care planning, and supervision of care, coordinating with the care team to manage symptoms and support patient goals. The successful candidate will work weekends (Friday–Sunday, 8am–9pm) with training Mon–Fri to start; a $15K sign-on bonus and Day 1 benefits #J-18808-Ljbffr
