About the Role

As an Licensed Practical Nurse (LPN) at Shippenville Nursing and Rehab, you will be responsible for providing direct patient care to our residents, including administering medications, performing vital signs, and assisting with activities of daily living. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring that our residents receive the best possible care and support.

Responsibilities

• Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with activities of daily living
• Assist with admission, transfer, and discharge processes, ensuring a smooth transition for residents
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Monitor and report changes in resident condition and needs to the healthcare team
• Maintaining accurate and detailed records of resident care and services
