Role Description The Case Coordinator is a full‑time, on‑site role based in Brooklyn, NY. This role is responsible for coordinating home care services for clients, including conducting intake assessments, verifying eligibility, and ensuring appropriate service plans are developed and implemented. The Case Coordinator will maintain regular communication with clients, families, caregivers, and clinical staff to monitor progress, address concerns, and update care plans as needed. Daily activities include managing case documentation, liaising with insurance providers and referral sources, and ensuring services comply with organizational policies and applicable regulations. The Case Coordinator will also support clients in navigating community resources and advocate for services that promote safety, independence, and quality of life. Qualifications Demonstrated skills in case management and coordination of services for elderly or disabled populations. Background in social work or related human services field, with the ability to assess client needs and connect them to appropriate resources. Strong communication and customer service skills, including the ability to work effectively with clients, families, and interdisciplinary teams. Experience working with insurance processes, authorizations, and benefits related to home care or health services. Knowledge of Brooklyn community resources and support services is preferred. Ability to maintain accurate records, manage multiple cases, and use basic office and case management software. Relevant education such as an associate or bachelor’s degree in Social Work, Human Services, Psychology, or a related field; equivalent experience will be considered.
