Your Role:Evaluate patients’ physical function and mobility in their homes.
Develop and implement personalized treatment plans to improve strength, balance, and independence.
Provide patient and caregiver education to support safe and effective care.
Collaborate with nurses, therapists, and physicians to ensure coordinated treatment.
Maintain accurate and timely clinical documentation.
Active Physical Therapist (PT) license in the State of Arizona.
CPR certification and valid driver’s license with reliable transportation.
Excellent communication, assessment, and problem-solving skills.
Home health experience preferred — but we welcome new graduates eager to learn!
