Key Responsibilities\n

• Conduct comprehensive evaluations and assessments for children enrolled in early intervention programs.

• Collaborate with families to develop and implement Individualized Family Service Plans (IFSPs) that reflect the unique developmental needs of each child.

• Monitor progress and outcomes as an integral member of a multidisciplinary team, ensuring that evidence-based practices are applied consistently.

• Establish rapport with children and families to create a supportive and empowering learning environment.

• Utilize the coaching model to deliver interventions in natural settings, promoting generalization of skills.

• Provide detailed information and skills to families, enhancing their capacity to support their child’s developmental progress.

• Stay current with federal and state regulations to assure compliance in service delivery and documentation.

• Commit to regular communication with other team members to ensure a cohesive approach to care.

Patient Population / Client Focus\n

Your primary focus will be on young children with disabilities or developmental delays aged from birth to three years. You will work closely with families in navigating the challenges they face while supporting their child’s growth and development. This role is crucial in advising families on resources, programs, and strategies that can help their children integrate and thrive within their environments.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Must hold a valid LCSW license issued by the Department of Public Health.
