Key Responsibilities\n

• Utilize your advanced social work education and training to conduct independent psychosocial assessments, diagnosing mental health concerns and shaping treatment plans tailored to individual patient needs.

• Collaborate with patients to create actionable, patient-centered treatment plans that encompass their physical, emotional, and social well-being.

• Employ evidence-based methodologies such as solution-focused therapy, behavioral activation, and motivational interviewing to facilitate impactful behavioral interventions.

• Work in close cooperation with a multidisciplinary team—consisting of clinical psychologists, primary care providers, and other social workers—to ensure a comprehensive approach to patient care.

• Monitor and assess patient progress through regular check-ins (both virtually and in person), tracking their responses to treatment and making necessary adjustments to achieve desired outcomes.

• Assist in the management of psychotropic medications by monitoring adherence, evaluating side effects, and assessing the overall effectiveness of the prescribed treatments.

You will primarily be working with patients facing complex health challenges associated with chronic or terminal illnesses. This includes providing emotional support and coping strategies for patients and their families as they navigate the intricacies of end-of-life care. Qualifications\n

• A Master's degree in Social Work from an accredited program.

• A current, valid, and unrestricted license to practice independently in mental health in the state of Washington as a Licensed Independent Clinical Social Worker (LICSW).

Compensation and Benefits\n
