Job Description
Title: Registered Nurse - Hospice

Schedule: Varies, rotating weekends

Assignment Details:
- Guaranteed Hours: 40
- Contract Length: 13 weeks

Requirements:
- Current unencumbered Oregon RN Licensure
- Healthcare Provider Level BLS certification
- BSN and/or previous home health/hospice or case management experience preferred
- Previous broad-based nursing experience preferred

Roles & Responsibilities:
- Provide nursing care to patients in home and facility settings.
- Coordinate with interdisciplinary teams to develop care plans.
- Monitor and manage patients' symptoms and medication.
- Offer emotional and spiritual support to patients and families.
- Document patient care and maintain compliance with healthcare standards.
- Participate in ongoing education and training to enhance skills and knowledge.

Compliance / Notes:
- Use of personal automobile required; must maintain valid driver's license and auto liability insurance.
