For more than 13 years, we have built a strong reputation among both clinicians and long-term care communities by going above and beyond to help facilities provide the best possible healthcare to their residents while remaining people-first in everything we do.As a Primary Care Provider, you will provide primary care to patients in long-term care facilities.This includes:Developing and implementing personalized treatment plans based on patient needsOffering medication management, including prescribing and monitoring medicationsCollaborating with a multidisciplinary team of healthcare professionals to ensure patient careMaintaining accurate medical documentation in line with regulatory standardsProviding education and support to patients and their families regarding mental health conditions and treatmentsQualifications:Current Nurse Practitioner license in specified locationAt least two years of experience is requiredExperience providing care to geriatric populations in long-term care settings is preferred but not requiredExcellent communication skills, especially when working with facility staff and residents' familiesAbility to manage time effectively, including flexibility in scheduling and being responsive to patient needs
