Care Manager

Job Description Job Description The Care Manager is responsible for assuring the quality of service provided to team members, coordinating the day-to-day operations of the team, and recommending and implementing changes that would improve the delivery of services. The Care Manager works with a panel of high-risk patients, one-on-one, to develop and manage their individual care plans. S/he is accountable for coordinating care in partnership with the care team and across the care continuum. This includes consideration of social risk and other issues. TASKS & RESPONSIBILITIES Management of beneficiary needs during transitions of care. Management of rare diseases and high-cost procedures. Consult with identified professionals, family members, and others, include their input in the Person-Centered Planning process, inform all involved stakeholders, and monitor the status of the consumer relationships in relation to treatment goals. High-risk care Chronic care management Management of high-risk social environments Identification of beneficiaries in need of care management Development of care management assessments/care plans Development and deployment of prevention and population health programs Coordination of services Documentation and documentation management/monitoring Fully participates in agency outcome evaluation system. Ensures the collection of required data Care delivery monitoring Coordinates and participates in clinical case conferences, when needed. Adheres to the Professional Code of Ethics and complies with the State of North Carolina Division of Mental Health, Developmental Disabilities, and Substance Abuse. Coach staff in working with consumers and provide training to other employees by demonstration, repetition, modeling, large group training, etc. Responsible for the development, implementation, monitoring, and revision of the Person-Centered Plan (PCP) as it relates to the needs of the individual Assist with the coordination of new admissions and transfers (e.g.) screening applicants and facilitating placement, etc. Assist in audits if needed Other duties as assigned KNOWLEDGE, SKILL & ABILITY Ability to support the mission, vision, and values of the company and abide by applicable standards of conduct, policies, and procedures. Ability to interact effectively with people from diverse backgrounds. Ability to communicate effectively, verbally, and in writing. Ability to problem-solve issues. Expertise in working with individuals who have chronic mental illness and/or co-occurring mental illness and substance abuse disorders Knowledge of and ability to implement the empowerment model Leadership skills that result in a positive work environment for staff and members Ability to work as part of a comprehensive community mental health team and to represent the facility in the community. Must be honest, dependable, and able to meet deadlines. Must be organized and able to handle multiple tasks. Self-motivated and able to work independently. EDUCATION & REQUIREMENTS A bachelor’s degree is required in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area, or licensure as a registered nurse (RN). At least 18 years of age Graduated from a four-year college or university Able to read, write, understand, and follow directions No substantiated findings of abuse or neglect listed on the North Carolina Health Care Personnel Registry Knowledge of Client Rights Ability to handle crisis/emergency situations, and utilize conflict resolution skills Knowledge of basic computer and applications including Microsoft Word EXPERIENCE Two years of experience working directly with individuals with behavioral health conditions or with an I/DD or a TBI need.
