Nurse Practitioner

We are recruiting on behalf of a confidential client for an experienced Nurse Practitioner to join a specialized care team serving frail and elderly patients across nursing home and community-based settings in Erie County, NY. This is a Monday through Friday role with a daytime schedule, offering stability, meaningful work, and a compensation package that stands out. What the role involves Conducting assessments, developing and managing care plans, and collaborating with an interdisciplinary team to coordinate care for a vulnerable patient population enrolled in an Institutional Special Needs Plan (ISNP). Rewards Package Base salary of $120,000 to $135,000 $10,000 sign-on bonus $10,000 annual performance bonus Up to 4 weeks PTO to start, with growth over time, plus 9 paid holidays Health insurance available within 30 days, base plan fully covered for the employee plus a $1,200 HRA contribution 100% employer-paid dental and medical malpractice coverage $1,500 CME allowance plus 3 paid CME days per year 401(k) with company match, FSA, life insurance, and EAP Who we are looking for A licensed Nurse Practitioner in New York with prior clinical experience.
